After Hours Acute Care

  • What's needed before moving to senior living. We have electronic Standard forms as well as building-specific forms.

  • You quickly fill as much as you're able, click submit. We review and confirm details, then quickly send to the community.  The document is printed to paper from a .pdf file to be kept with the community.

  • Pre-move in video visit:

      To avoid delays, we can do a video         visit from anywhere  

  • Issues are often unstable. We can anticipate needs, trigger resources, have meds and orders in place, and fill out forms before move in.

  • Below are some community specific forms, and we can digitize for any.

  • What happens after a person has died.  Code status only applies when one has stopped breathing and tells others whether to try to bring them back.  If this isn't on file, the default is to try, with chest compressions (broken ribs), electric shock, and over 80 years old, causes suffering and is rarely successful.

  • Most people would rather rest in peace when it's their time. Consider what that person's values are.

  • Now, before the crisis happens, be sure to download and print, have it notarized, and on file with us and where they live.

  • What happens now, before a person has died.  It guides the healthcare team to be more or less aggressive.

  • As age advances and circumstances change, it's important regularly consider and reconsider a person's considers quality of life and adjust our care plan. 

  • If someone wants to fight, we'll adjust meds, engage more resources, more specialists, more therapy. We're all working and communicating on their behalf.

  • There may come a time where these measures do not improve quality of life, or a person isn't recovering after hospital visits.

  • Take time now to put it on paper, and re-review directives at least once yearly and after hospital visits.
